Damon & Carlton have said the sideways world is not an alternate reality. OK, that's kind of cheating, but what else could it be?
I think it is a lie. It seems like the past has been rewritten, but mainly because there are so many dead people that are alive. But these are all people that have died on the island and we know they can manifest somehow. I think MIB has won and populated the real (sideways) world with dead people from the island.
Sideways Locke believes he is responsible for his father's condition, but he doesn't remember it. I think it didn't happen.
Jack doesn't remember his apendectomy because I don't think it happened.
People in the sideways world are remembering the island, which means the island is in their past.
There is only one person in the sideways world that doesn't belong...David. He is MIB. He won and the sideways world is his solution to getting off the island. The question then is what will happen when everyone remembers?

Here's the deal - in my opinion. Now that Jack has the power - he has the means to set things straight - at least as far as he sees things. Remember back to when he bought into the plan that the Jughead bomb would fix everything - would make sure that the crash never happened - well this is going to be his chance to finally make sure that it doesn't happen.
I Originally thought that what we saw in the "Sideways" world was the result of every one being sent back to the point in their lives when Jacob touches them - but now they live their lives as if Jacob had never come to them. This works for just about everyone - except for Hurley and Sayid - Jacob came to them after they had returned from the island - but then again - maybe that's the only time we were shown that he was in their lives. Maybe he was there for Hurley's birth - or in some Iraqi prison.
Anyway - the whole point is that I think Jack puts everything back to rights.
Now - why Desmond is trying to get them to all remember their "other" lives - their island lives - is beyond me.
