Obviously since Jack seems to not recall the appendix scar it is an indication that it is just one of the indicators that the Lostees know something is not quite right in the flash sideways.

Is it a coincidence - that when Claire axed Justin it seemed to be where his appendix would be?

Another thought in the alternate reality where Jack kept his appendix was it because Jacob was watching over him?

Well his appendix is gone in both timelines. It's just when that changed.

I think Jacob caused Jack appendicitis on the Island to keep him from leaving. Although he did anyway.

This adds a really interesting development that things that happened on the island need to sync up somehow in the flash sideways.
